Go to "my games and app", select "full library" on the left hand side, then find the tsw5 icon and select install
Although actually, press "manage game" once you've found tsw5 then you can select the base game and which add ones you want to install too
If you log onto your new Xbox with your existing profile, everything will be there from your old Xbox
