Any failure should be eliminated anyway in order to continue your service.

It is annoying when a failure occurs resulting in a "game over"

An major example is the SPAD or Signal Failure. In real life when you skid through a red signal it is not game over. Why then does this show up in TSWx passing a red signal at danger. Running a red signal in TSC does not result in a "game over" when in standard mode or in free roam mode so why does this happen here in TSWx

The same with derailments. On derailment your service ceased at once without viewing the results for some period. To go further. Where is the rerailing service and equipment. Shouldn't it be wonderful to have a train back on track using rerailing equipment with a heavy 500t rail crane, beams and jacks. This would be some scenario I tell you.

As far as your job is concerned, it probably is Game Over.

In real life, running a red would be service over pretty quickly, in 95% of cases, even if you'd have to get to a station where you can be relieved, and then you have the dreaded no-tea-or-biscuits meeting with management which is, touch wood, not something I have experienced, though it is apparently not so awfully nice. In TSW though, I actually wouldn't be surprised if licensing has something to do with it.

Not necessarily, I know various drivers who have had SPADs and still driving. It is very circumstantial as no two SPADs are the same.

I’m assuming that’s a core limitation because a SPAD (or derailment) breaks the timetable. The train is now in a place or state which it isn’t allowed to be, so the simulation falls apart. Could also be abused for some nasty deliberate incidents/crashes.

I like the idea, but it's not necessarily worth the hassle to add this beyond a few scenarios.
As far as SPAD goes, I could see how that could maybe be reworked, of course the concern is that someone's going to abuse it badly and recreate horrible incidents for the "Fun of it".

However, when it comes to derailing. I think it's fair to say that if you have derailed, you wouldn't be very inclined to go back to the site of the incident. Adding re-railing equipment to re-rail your train during timetable mode would break a good couple 5 or 6 things.
It could be added as a scenario, but then again, it's up to DTG and how it would fit the route DLC.

Tbf if my train derails it is very unlikely I am going to wait an age for the re-rail equipment to show up. I am very much quitting the game.
