I've had a lot of issues with brakes in TSW, both in the US and Euro trains.
The most frequent occurrence is that it will appear that brakes have been applied but according to all the gauges and me holding down release they should not be.
The train will still slow down to a halt rather quickly instead of coasting even while going down hill.

On the flipside I've also had the brakes appear to be non functional, so far this has only happened on the US line.
Even with the brakes in full service the train will hardly slow down (uphill/downhill makes no difference) until I throw it in emergency and then all of a sudden the train will start to slow down properly. All 3 brakes (automatic/loco/dynamic) will have no to little effect on the train unless maxed out.

Brakes need some tuning, that's for sure, especially for US freight, but another way around - they're too responsive.

Though the only thing i can call a real glitch sometimes occurs with 166, when you're taking over the service from AI driver that goes in the wrong direction (virtual tail forward). Then on rare occasion safety systems in another cab can't be disabled fully and all of a sudden decide to apply brakes, then realize its mistake and release them, only to apply it again minute later. But that's all I've encountered for ~30 completed services.

Both of your cases look more like incorrect configuration and/or usage of safety systems (first one) and incorrect brakes configuration for MU or DP operation (second one)
