So I dunno how representative this is, and if it's meant to be the case, but I was running the 16:11 NWP-GCT and uh... all of a sudden as I'm heading down the track near Fordham on a green light, there's a train sitting on it in front of me. No warning, derailment since not even the emergency brakes can apply in time.
I've noticed on this route that it'll often give me a yellow when the train in front is just a few yards away, and I have been keeping pace with it, but this is the first time it's been green and I've had one parked in front of me.

Is this intentional? Is this something you're meant to just keep an eye out for on this route? I was going about 10mph under the speed limit but don't know how I could have reacted in time even at half that.

I'm used to signal errors when reloading a save but this was a straight single session run.

Are you driving with safety systems on? Apparently this route allows more than one train to occupy one signal block, so you need to watch the safety systems, as they will tell you to slow down, indicated by the small lights displaying letter like N, M, R, etc.

ahhhh, cool. This would explain it. Was hoping for clarification if this is how the systems on the route operate, or a bug, good to have an answer promptly.

Yeh it's not a bug. Even with the safety systems off, you still gotta pay attention to your signal speed.

Looks like those passengers onboard that train may file a lawsuit against you now and the MTA will have to fire you for not paying attention :)
