It would require a lot of work as seen in sehs extended. The timetable had to be remade, existing track and scenery had to be updated and new track had to be made. Along with various upgrades such as adding in approach control which wasn't something that is required for an extension but a new feature regardless

In tsc, you didn't have to worry about timetable mode, so the process was much easier.

Extensions could add more locos to routes.

Add the UP Palmdale Cutoff-West Colton Yard and the Barstow-Yermo branch to Cajon Pass. Loco DLCs could include a UP SD70M, C45AH, C45ACCTE, GP60/GP62, or GP40N. A BNSF B40-8W, C44-9W, ET44C4/ET44C6H, or GP60/M/B would be great loco DLCs for the BNSF part of Cajon.

Peninsula Corridor could be extended to Gilroy, and a UP GP60/GP62, SD70M, or GP40N could be added for more local freight operations.

The new ECML could give us a Class 55 Deltic with a future extension. Maybe an LNER A4 Pacific could also be added.
