In all seriousness, it's not really right on a product that is played by many people who work in Or retired from) the rail industry and sadly have to confront this for real either directly or indirectly, from time to time.
Sadly, it happens on a lot of routes where the NPC either fall through the platforms onto the tracks, or fall out of the train and onto the tracks when getting off. Even had an AI passengers walk off the platform edge in front of my train a few times on Hamburg-Lubeck. DTG need to fix AI passenger path boundaries, and repair the holes in platforms to stop AI passengers falling through them on all routes.
Saw them on GWE sometime ago, I think on the slower rail side, notable as it wasn’t exactly near a station if I remember correctly.

This occurs on multiple routes, either because they spawn there or due to the large volume of passengers, they push each other over onto the tracks. At least from what I've seen.
Clearly this is an undesirable bug for obvious reasons, but at this point I view it as a "random event" DTG/Sam was exploring at some point. Had to use my emergency break and a lot of horn coming into Ruggles station the other day...
