Thanks guys. I guess it does have to do with the Windows patch. Ticking "disable all sound effects" in my Realtek settings seems to have fixed it.
Ok then it is an OpenAL issue with the 64bit OpenAL. The 32bit openAL doesn't do this but it doesn't work with Railworks64.exe

Good config but some interior and dial lights don't work with it like the class 142 by AP
All of them do it for some reason which leads me to believe it is a sound engine issue.
The game engine actually goes back to MSTS.
Switching to Shift+2 (headout left/right) view kills all engine sounds until you swivel the camera backwards towards the body of the loco. Engine...

It fixes itself after a while. I have to go up real close to the tracks or start moving for about a minute. The textures don't load for a while....
