What does actually load up when you start a scenario like this? But to answer your original question, people have been able to mod custom 3D models into the game for quite a while now. Of course they are not functional, but can be great for screenshots. I assume they took the screenshots and uploaded a custom scenario with those screenshots.
While Model changing/loco mods are a thing now (one is avaible at trainsimcommunity, the other ones are just Images of unpublished mods), under no circumstances they could be shared on CC, so the Pictures are just clickbaiting. And DTG would leak it in many other ways, but not of a Screenshot of a mediocre model (If it'd be a real addon, the work of the modders is of course impressive) on Creators Club.
