Never had this happen before, but then I only just got the HFP. In the scenario "Gronk and Whistle" a light turns red without warning. It's the exit light from the Morley Tunnel (eastbound): yuou pass the approach light, it's green, all is well; the HUD reports that the exit light is green also. But then, mid-tunnel, the light goes red. (if one is playing without HUD, or as I do use the HUD as advance notice while still driving according to what I can see, this is lethal, because the signal is located around a bend and not visible until within ~300 yards: not a prayer of stopping, unless you have telepathically started braking before you can see it.) And there is another signal farther on towards Leeds which does the same- turns red after you have passed the preceding signal with a green aspect.
