This issue has started happening after the latest update on Tuesday 16th, after which all the ICE services on SKA now have the cab car with 2nd class at both ends of the train instead of only the rear end. I assume it's an issue with the spawning vehicle which was also present on Rivet Games' Isle of Wight. I don't know how to show the bug but I hope these two screenshots can clarify what I'm talking about. Front: Rear:
