There is a new update for UE4, version 4.18. Is this used only in creation or will it affect game play?

Won't affect gameplay at this time, but some of its features and perks will be used to improve existing and upcoming content at a later date :)

If I was DTG I probably wouldn't update it at the moment, unless there was a specific feature(s) in the newest version of UE4 that was really useful for future development. It could be a lot of work to update it as it's inevitable that it'll break something, and TSW isn't all that reliable on the existing version as it is.

Plus if they're fairly far down the road with Scenario/Route/Asset Editor and Multiplayer aspects of the game, they should probably complete development of those first before updating to a newer UE4 version. If you update midway through development you can end up trying to figure out whether a bug is a problem with your code or a problem with the update. At least if you get a stable build using an older version, you know any issues after the upgrade are related to the upgrade.
