They're not present on all routes or at all stations. One picture of passengers standing at a platform on one route isn't evidence they exist...

The testing team aren't at fault. It's whoever decides to release the route in its current state who are the problem.

It isn't. High and low tones are used for different purposes. They can be played one after the other, but not both together.
